  In-home care from Sarah & Jacobs Home Care Health Services allows your loved one to maintain their dignity and independence.  This experience gives individuals  the ability to live longer, more independently in the comfort of their own home surrounded by family and friends.

Recovering from home after surgery can limit you with your daily activities. Whether you are recovering from major surgery, childbirth, or plastic surgery, our compassionate caregivers can assist you to a healthy and complete recovery.

Arthritis, physical disabilities, and age-related conditions are some of the few circumstances that prevent patients from caring for themselves. Our caregivers can assist you with daily tasks such as mobility, eating, exercising, and grooming.

Our experienced and highly trained caregivers provide the highest quality of care to both patients and their families. Our goal is to help individuals and their loved ones live happy and fulfilling lives. 

  Consumer Directed Services program allows individuals suffering from an illness the opportunity to "hire" their own personal care attendant to provide their care.  Consumer Directed Services often are less stressful and easier to adjust to, than moving to a nursing facility. 

 Sarah & Jacobs Home Care and Business Ent LLC. Consumer Directed Services program is a great way for a family member or friend to get paid to take care of their loved one in the comfort of the loved one’s home. This program enables your loved one to continue living independently while receiving assistance with activities of daily living such as: bathing/showering; toileting; obtaining/organizing medication; meal preparation/clean up; housekeeping/laundry; and essential transportation.   

  Skilled nursing care is a primary service offered by Sarah & Jacobs HomeCare and is performed by a professional registered or licensed practical nurse under the following guidelines:  

• The registered nurse or Licensed Practical Nurse makes the initial evaluation visit within 48 hours of receipt of the referral, and regularly reevaluates the patient’s nursing needs for necessary revisions.
• The  nurse initiates the plan of treatment and provides those services requiring substantial specialized nursing skill, along with preventive and rehabilitative nursing procedures.
• The nurse prepares clinical and progress notes, and informs the physician and other personnel of changes in the patient’s condition and needs.
• Coordination of services are performed with other health care disciplines along with participation in in-service programs.
• The nurse counsels the patient and family in meeting nursing and related needs and supervises and teaches other nursing personnel.

Sarah & Jacobs Home Care and Business Ent LLC provides private duty

care. Private Pay is home-health care as defined in all service areas mentioned above and provided by our home care team. This service is  paid either out of pocket or through private health insurance.  At this time, there is no supplement from the government program to fund private care.
